Fill out one form per student (entering Grades Pre1A and above.) Different forms are required for the Elementary Boys and Bais Yaakov.
3) Family Information / Consent Form
Current Families: Your personalized form was emailed to you via Infograsp as part of your Re-registration email on January 24, 2022.
New Families: Please refer to your acceptance / registration packet.
When editing the document, please be sure to clearly mark your edits (an asterik, colored ink, etc.)

FACTS (Tuition Collection Plan)
Both tuition and supplemental donations will be collected via FACTS. Click Here to access.
Current Families: Log onto your FACTS account and ensure that your payment plan is set up correctly for the 2022-23 school year.
New Families: Set up your tuition payment plan via FACTS (Click Here for detailed letter.)
To apply for a scholarship, log onto your FACTS account and select "Start Application'. Please complete and submit your application by May 2, 2022.
Medical Forms
Please note: These forms are not required for registration but are needed before the Admission Card Deadline in late July.
